Position Purpose: Assertive Community Treatment (ACT) Team is recovery and resiliency oriented, intensive, community based rehabilitation and outreach. It is team based and consists of mental health rehabilitation interventions and supports necessary to assist the recipient in achieving and maintaining rehabilitative, resiliency and recovery goals.

ACT is designed to meet the educational, vocational, residential, mental health co-occurring disorders (MH/SA, MH/DD, MH/Medical), financial, social and other treatment support needs of the recipient. Interventions are provided in natural settings, and are delivered face to face, by telephone, or by video conference with individual recipients and their family/significant others to the primary well- being and benefit of the recipient.

Qualifications/Skills:

  • Master degree in counseling, psychology, social work or another related field.  Illinois clinical license is preferred LPC, LSW.
  • Prefer experience in supervising a team in a social service setting.
  • Ability to motivate and problem solve with staff.
  • Ability to work with persons from diverse backgrounds and personalities.
  • Use effective crisis intervention approaches to reduce the impact of increased symptoms on the client and their care taker.
  • Skills to adapt verbal and written communication to the language style of the client and their family.
  • Ability to adequately assess and re-assess diagnosis, functional limitations and develop plan for reducing these limitations.
  • Ability to coordinate services from a variety of community resources.
  • Good keyboarding skills and understanding of Microsoft Office Suite, ability to learn new computer programs easily.
  • Positive demeanor, empathetic and calm.
  • Valid driver’s license

Essential Responsibilities:

  1. Work scheduled hours and any additional hours deemed necessary by leadership. Some variation in scheduled hours may be required to meet company needs.  
  2. Use available data to monitor effective team behavior in compliance with standards.
  3. Provide staff feedback on performance via team meetings, monthly supervision and annual reviews. Provide more frequent individual feedback when needed
  4. Participate as part of the management team providing support to the team and other departmental staff
  5. Complete all necessary monthly and quarterly reports by appropriate time lines
  6. Participate in the on-call system
  7. Complete all clinical documentation based on all appropriate regulatory requirements and Rosecrance policy
  8. Utilize variety of treatment concepts, evidence based practices and service modalities when providing care
  9. Demonstrate an ability to identify an individual in crisis, reduce the impact of the crisis on the individual and their supports and determine appropriate disposition
  10. Provide leadership by demonstrating self-motivation, seeking improved performance, problem solving while meeting the needs of the staff and consumer
  11. Serve as a member of the team, and participate in all team meetings, duties and activities as assigned.
  12. Exhibits actions and makes decisions that align with the Rosecrance Code of Conduct and within the framework of the law.
  13. Perform all responsibilities in compliance with the mission, vision, values, and expectations of Rosecrance.
  14. Demonstrates strong customer service skills that incorporate the 5 tenets of the Rosecrance Way into daily practices and interactions with all stakeholders.
  15. Serve as a role model to others and demonstrate excellent company stewardship.

Schedule: Monday-Friday 8:30AM - 5:00PM

Pay: Based on education, experience, and credentials
